Highly acute patients often come to Evolve directly from the hospital. Whether your patient struggles with suicidality, self-injurious behavior, psychosis, overdose, or any other life-threatening mental or behavioral health crisis, Evolve ensures a smooth, efficient admission. 

When referring a patient from the hospital, please follow the steps below: 

Step 1: Fax Clinical Packet 

Please fax the patient’s Clinical Packet to Evolve. 

The Clinical Packet should include: 

  • Face Sheet 
  • Insurance Information 
  • Recommended Level of Care 
  • Initial Psychiatric Evaluation 
  • Progress Notes 
  • TB Test 
  • History and Physical 
  • Medication List 
  • Lab Work 
  • Vitals/CIWA/COWS

Step 2: Call Admissions 

Call one of Evolve’s admissions coordinators to ensure we received the clinical packet. 

Let our team know how you would like us to proceed, and who on your team we should contact for next steps.

Step 3: Maintain Contact 

To ensure continuity of care, please maintain an open line of communication throughout the patient’s remainder of stay at the hospital. If there are any pertinent changes to the patient’s treatment progress or discharge plan, please update our admissions team accordingly.
